  • Alternative Care (AC)
A state-funded program for people not financially eligible for Medical Assistance, but who meet AC financial and service eligibility requirements that supports limited home and community-based services for people age 65 years and older, who require the level of care provided in a nursing home.  People eligible for Alternative Care have low levels of income and assets but are not yet eligible for Medical Assistance.  The purpose of this program is to promote community living and independence with services and supports designed to address each person’s individual needs and choices.

  • Personal Care Assessments (PCA)
Assessments of financially eligible persons with disabilities or special health care needs requiring assistance with personal care to determine level of care and provide information on care options.
