The Adult & Disability Team is here to work with adults and their families who are vulnerable, with a disability or other significant barrier to safety, permanency, or well-being, to have timely access to financial and health care assistance so they can be safe, have permanency, and be well.

The Adult & Disability Unit is responsible for providing services and case management to people in the following program categories:

  • Consolidated Chemical Dependency Treatment Fund Adolescent and Adult Rule 25 Chemical Dependency Assessments and Treatment Referrals
  • Rule 24 Financial Eligibility Determination for the CCDTF
  • Pre-Commitment Screenings, Referrals, and On-Going Case Management Services
