Death Certificates

Our building hours are 8:00 am to - 4:30 pm.

Dodge County Recorder's office is able to process death certificate requests when the death occurred in any County in Minnesota during or after 1997.  If the death took place before 1997, you will need to contact the County where the death occurred or order by mail from the Minnesota Department of Health.
Death Certificates can be obtained on the same day if you apply in person.  You must be an Authorized Requester, have valid identification, and complete a Death Certificate Application.

An Authorized Requester is determined by the Minnesota Department of Health.  Please click on Authorized Requester  for a detailed list from the Minnesota Department of Health.     

If you are mailing your request for a Death Certificate, your signature on the out of office Death Certificate Application must be witnessed by a Notary Public.
