Birth Certificates

Our building hours are 8:00 am - 4:30 pm. 

Birth Certificates can be obtained on the same day if you are applying in person.  You must be an Authorized Requester, have valid identification, and complete a Birth Certificate Application.

An Authorized Requester is determined by the Minnesota Department of Health.  Please click on Authorized Requester for a detailed list from Minnesota Department of Health. 

If you are mailing your request for a Birth Certificate, your signature on the out of office Birth Certificate Application must be witnessed by a Notary Public.

Wrong information on Birth Certificate?
Some changes can be made if the subject of the record is under 1 year old, and a certificate has not been printed yet.  If the subject is older than one, please visit the Minnesota Department of Health web page for the record amendment packet and information.
