- Elderly Waiver (EW)
A program
that funds needed services for people over 65
and older that are eligible for medical assistance
and require the level of care provided in
a nursing home, but choose to reside in the
community. The purpose of this program is
to promote community living and independence
with services and supports designed to address
each person’s individual needs and
choices.
- Alternative Care (AC)
A state-funded program for people not financially eligible for Medical Assistance, but who meet AC financial and service eligibility requirements that supports limited home and community-based services for people age 65 years and older, who require the level of care provided in a nursing home. People eligible for Alternative Care have low levels of income and assets but are not yet eligible for Medical Assistance. The purpose of this program is to promote community living and independence with services and supports designed to address each person’s individual needs and choices.
- Personal Care Assessments (PCA)
Assessments
of financially eligible persons with disabilities
or special health care needs requiring assistance
with personal care to determine level of care
and provide information on care options.
- Long Term Care
Consultation
Long-term care consultation (LTCC)
services assist persons to make long-term care
decisions through the selection of service options
that meet their needs and reflect their preferences.
Information provided is intended to prevent or
delay nursing facility placements and provide
transition assistance after nursing facility
admission so a person can return to the community.
